Medication Refills:

If you are in need of a medication refill, please contact our office at 480-491-1898.  In order for us to process your refill request please have your pharmacy name and phone number as well as the medication to be refilled. Please be advised that refills may take anywhere from 4-7 days. Certain medications will not be eligible for a refill without a follow-up visit.


Prior Authorizations:

Some insurance companies require prior authorizations for certain medications before they will cover them. Prior authorizations can take up to two weeks.  Depending on your insurance coverage, prior authorizations may be required for certain prescription medications.

Letters & Forms:

If you have a form that needs completed or a letter that needs to be sent regarding your treatment, please contact our office at 480-491-1898. Please be advised that a fee will be assessed for the completion of these forms.

Request for Medical Records:

We are committed to protecting the privacy HIPPA Privacy Policy Notice of your medical information. To safeguard your privacy, you must complete and sign a form before records can be sent. On the form, you are required to indicate what records are to be released and where they are to be sent. Although we make every effort to get your records out as quickly as possible, please be advised that this process may take up to 30 days. Insurance & Fees

Fees are subject to change at any time

  • Missed and cancelled appointments with less than 24-hour notice will be charged to you directly as insurance companies typically do not pay for these charges. Payment will be required for missed appointments before another appointment will be made.
  • Fees are collected at the time of the service.
  • Checks are only accepted from established patients and at the Treatment center’s discretion.
  • Transitions Center is not an in-network provider for Medicaid or Medicare. Some of our clinicians may not be an in-network provider for your insurance plan.  Please check with our staff to see if one of our providers is an in-network provider for your insurance plan.
  • Transitions Center does provide treatment services for most insurance plans which offer “out of network” benefits. Please call our office to see if your insurance plan is one of them and if we can provide you services under that category.
  • Telephone consultations and e-mail correspondence will be billed just like regular clinical service; please see our fee schedule below in this regard.
  • Please review the Financial Agreement as you will be required to sign this prior to receiving clinical services from our providers.
  • Transitions Center does offer discounted fees for many programs and services for patients who are on cash/self-pay basis. This is when patients either do not have insurance benefits or for some reason do not want to use them. Please check with our staff to find out about these, often deeply discounted, cash rates/fees
  • Transitions Center also runs special promotions on select treatment programs and services, usually on a time limited basis, for self/cash paying patients. Please call our office to find out about our current promotional programs and offers.

“Urgent” Matters

Transitions Center DOES NOT have providers who are “on-call” after hours and as such it provides services ONLY during regular business hours. If you are a currently receiving services at  Transitions Center and have an urgent matter that you believe cannot wait until the following business day it is likely that your situation is emergent and would be better served by calling emergency or crisis services listed above. However, you can call during normal business hours to request an “urgent” appointment which will be evaluated by our staff and accommodated based upon the nature of the situation and provider availability.Please note that it is the patient’s responsibility to request refills at least 5 business days before running out of medications. Refill requests are only evaluated during regular business hours.
