Transitions Center was founded and is managed by a triple Board Certified psychiatrist of high repute who has lived and worked in the Phoenix area for over 25 years. Our providers excel in providing compassionate and individualized care while treating patients of all ages with different types of psychiatric disorders and alcohol and substance abuse issues. Dr. Neff completed his undergraduate degree in Physiology Magna Cum Laude from University of Arizona and graduated the top third of his medical school class at Tulane University School of Medicine in New Orleans, Louisiana. While at Tulane, he completed his Master’s in Public Health in Health Systems Management at the same time as earning his Doctor of Medicine degree. During medical school, Dr. Neff participated in two medical foreign aid trips working in rural and underserved areas of Nicaragua with the international aid organization Bridges to Community.
He completed his psychiatric residency at Maricopa Integrated Health System in Phoenix, Arizona and is currently board certified by the American Board of Psychiatry and Neurology. He has worked treating patients at Aurora Behavioral Health since 2014 and served as the Aurora Director for Resident Services since 2015. Dr. Neff specializes in treating patients struggling with various psychiatric disorders such as anxiety, ADHD, depression, bipolar disorder, OCD, PTSD and various other mental health conditions.

Dr. Ghafoor completed his psychiatric residency at St. Elizabeth’s Hospital in Washington D.C., followed by two fellowships (Child & Adolescent Psychiatric and Forensic Psychiatry) at William S. Hall Psychiatric Institute, USC School of Medicine in Columbia, South Carolina.
Dr. Ghafoor is triple Board Certified: General Adult Psychiatry, Addiction Psychiatry & Forensic Psychiatry. He has also completed a 2-year accredited Fellowship in Child & Adolescent Psychiatry. He has been practicing in the Phoenix area for over 20 years and has served in leadership positions at various local psychiatric hospitals and is currently the Medical Director at Aurora Behavioral Health, Tempe. He is the Past President of the Arizona Psychiatric Society (2012- 13), and an Assistant Clinical Professor at the University of Arizona, College of Medicine Phoenix.

Yarmoni is a Board-Certified Psychiatric Mental Health Nurse Practitioner. She first graduated with a Bachelor of Science in Nursing from Grand Canyon University. As a Registered Nurse, Yarmoni gained a well-rounded experience working in multiple settings managing a wide range of diseases and disorders but was always drawn to mental health. Through personal and professional experiences, Yarmoni saw the impact of mental health on society and understood the need for mental health providers. As a result, she obtained a Doctor of Nursing Practice degree with an emphasis in Psychiatric Mental Health. Yarmoni has experience working with various psychiatric disorders in adults and adolescents above the age of 12. She is dedicated to educating and collaborating with her patients to achieve a mutual goal and ensure the best possible outcomes. She is committed to providing a safe and welcoming environment for patients to heal and gain the necessary tools to improve their mental health. She is eager to make sure that all of her patients feel heard and well taken care of. The quickest way to Yarmoni’s heart is through good food and music. In her free time, Yarmoni enjoys hiking, painting, making others laugh, and any activity that involves water.

Alexandra “Lexy” has been a psychiatric provider here at Transitions Center since July 2020, working with children, adolescents, and adults. Lexy also works part time at Aurora Behavioral Health Hospital in Tempe, primarily helping adolescents. Prior to attending PA school, she first completed a Master’s in Biomedical Sciences at Midwestern University in Glendale, AZ. Lexy completed her PA Master’s degree at Chapman University in Orange, California which is where she found her passion for mental and behavioral health, working in many settings to include drug and alcohol rehabilitation, acute inpatient, and adolescent psychiatry. Lexy is experienced in treating patients 10 and up.
During her free time, she enjoys being outdoors, exploring Phoenix’s growing culinary scene, and
spending time with her family and French Bulldog.

Delaney is a Board-Certified Physician Assistant with a strong passion for mental health. She was born and raised in Florida where she completed her Bachelor’s Degree in Health Science at the University of Florida in Gainesville before pursuing her Master’s Degree in Medical Science Physician Assistant Studies at Nova Southeastern University in Fort Myers, FL. During her years in undergrad she worked and volunteered in many healthcare settings including neonatal and assisted living facilities. It was during PA school when she really discovered her passion for mental health and the importance of increasing awareness about the importance of mental health. Delaney is experienced in treating patients ages 13 and up and is a Certified Suboxone Provider.
During her free time, Delaney enjoys exploring the outdoors with her dog and reading a good book. She is excited to experience all that Arizona and the west coast have to offer.

Oksana is a Board-Certified Physician Assistant who specializes in mental health and also works part-time in Aurora Behavioral Health Hospital in Tempe, AZ, primarily with adolescents ages 13-17. Oksana was born in Kiev, Ukraine and is fluent in Ukrainian, Russian and English. After high-school studies in England and upon undergraduate University Studies in Kiev, Ukraine, Oksana moved to the United States.
Oksana has completed her BS in Biology at Grand Canyon University and a Master’s Degree in Medical Science Physician Assistant Studies at Northern Arizona University. She has worked in various health care organizations, including the Mayo Clinic, and also enjoyed volunteer work along the way; especially helping disabled adolescents and their families. Oksana developed a true passion in mental health and various psychiatric disorders during PA school and ultimately decided to pursue this career in order to serve others who are in need.
During her free time, Oksana enjoys spending time with her husband, 2 adolescent children, and dog Buster. She likes to stay physically active by doing yoga, pilates and hiking. She also enjoys cooking Ukrainian food and singing karaoke
