About Transition Center

Transitions Center of Tempe, AZ specializes in the diagnosis and treatment of various psychiatric disorders such as Attention Deficit Hyperactivity Disorder (ADHD), Anxiety Disorders, Bipolar Disorder, 

Obsessive Compulsive Disorder (OCD), Depression, Insomnia, Panic Disorder, Post-Traumatic Stress Disorder (PTSD), Schizophrenia, Schizoaffective Disorder, Insomnia and Stress induced symptoms, as well as co-occurring disorders. We service Tempe and the greater phoenix metropolitan area.

Transitions Center was founded and is managed by a triple Board Certified psychiatrist of high repute who has lived and worked in the Phoenix area for over 25 years. Our providers excel in providing compassionate and individualized care while treating patients of all ages with different types of psychiatric disorders and alcohol and substance abuse issues. The center follows evidence based clinical guidelines and a holistic approach which leads to the most effective treatment of individuals suffering from various psychiatric disorders.

Meet Our Team

Ryan Neff M.D.

Dr. Neff completed his undergraduate degree in Physiology Magna Cum Laude from University of Arizona and graduated the top third of his medical school class at Tulane University School of Medicine in New Orleans, Louisiana. While at Tulane, he completed his Master’s in Public Health in Health Systems Management at the same time as earning his Doctor of Medicine degree. During medical school, Dr. Neff participated in two medical foreign aid trips working in rural and underserved areas of Nicaragua with the international aid organization Bridges to Community.

He completed his psychiatric residency at Maricopa Integrated Health System in Phoenix, Arizona and is currently board certified by the American Board of Psychiatry and Neurology. He has worked treating patients at Aurora Behavioral Health since 2014 and served as the Aurora Director for Resident Services since 2015. Dr. Neff specializes in treating patients struggling with various psychiatric disorders such as anxiety, ADHD, depression, bipolar disorder, OCD, PTSD and various other mental health conditions.

Tariq Ghafoor M.D.

Dr. Ghafoor completed his psychiatric residency at St. Elizabeth’s Hospital in Washington D.C., followed by two fellowships (Child & Adolescent Psychiatric and Forensic Psychiatry) at William S. Hall Psychiatric Institute, USC School of Medicine in Columbia, South Carolina.

Dr. Ghafoor is triple Board Certified: General Adult Psychiatry, Addiction Psychiatry & Forensic Psychiatry. He has also completed a 2-year accredited Fellowship in Child & Adolescent Psychiatry. He has been practicing in the Phoenix area for over 20 years and has served in leadership positions at various local psychiatric hospitals and is currently the Medical Director at Aurora Behavioral Health, Tempe. He is the Past President of the Arizona Psychiatric Society (2012- 13), and an Assistant Clinical Professor at the University of Arizona, College of Medicine Phoenix.

Dr. Ghafoor specializes in treating adults and adolescents with all types of psychiatric disorders as well as alcohol and/or substance abuse issues.

Channing Cabanillas PA-C

Channing is a third generation Arizona native and has always had a strong desire to work in the field of medicine. Channing graduated Summa Cum Laude with a Bachelors in Biology and a Pre-PA emphasis from Grand Canyon University. She went on to attend the Northern Arizona University – Physician Assistant Program at the Downtown Phoenix Biomedical Campus where she graduated with a Masters Degree in Physician Assistant Studies.

Channing has been a valuable member of Transitions Center’s clinical team since April of 2017. She specializes in treatment of all types of psychiatric disorders in adults and adolescents above the age of 14. She strives to provide compassionate care to all of her patients. It is highly important to her that each patient feels they are being treated as an individual.

In her free time, Channing enjoys traveling, live music and spending time with her friends, family and dog.

Elizabeth Suprak PA-C

Elizabeth is a certified Physician Assistant who completed her Master’s in education at NAU and Masters in Physicians Assistant at A.T. Still University in Mesa AZ. She also has two Bachelor degrees in Science and Education from Ottawa University. Although she has worked in primary care and family medicine, Elizabeth is keenly interested in treatment of psychiatric disorders.

While Elizabeth provides services to all age groups, her passion is to treat children and adolescents. She strongly believes that patient education is essential to achieving optimal outcomes. Elizabeth often tells her patients “it’s not just about medication management; let’s journey together towards finding the  unique and special attributes that makes you who you are, and then get you the tools to become everything you want to be”. Elizabeth is a Certified Suboxone Provider.

Elizabeth enjoys spending her free time with her family and grandchildren, and, maintaining a healthy lifestyle, traveling, cooking, and crafting.

For higher education, he came to the USA and earned his Masters in Physical Therapy at Oakland University, Michigan, specializing in Neurological Rehabilitation. His desire for further education in medicine brought him to Phoenix, Arizona, where he received his 2nd Masters from Northern Arizona University in Physician Assistant (PA) studies. He practiced in Primary Care and Physical Medicine & Rehabilitation before joining Transitions Center as a Psychiatric PA. His intelligence, eagerness to learn, dedication and unwavering commitment to excellent patient care along with his down to earth personality makes him a great asset to the center. He specializes in treating adults and adolescents (above the age of 14) struggling with psychiatric and/or substance abuse disorder.

Mani is a traveler at heart and likes to visit different countries to experience various cultures. He is also an avid motorcycle rider.
