About Transition Center

Transitions Center of Tempe, AZ specializes in the diagnosis and treatment of various psychiatric disorders such as Attention Deficit Hyperactivity Disorder (ADHD), Anxiety Disorders, Bipolar Disorder, 

Obsessive Compulsive Disorder (OCD), Depression, Insomnia, Panic Disorder, Post-Traumatic Stress Disorder (PTSD), Schizophrenia, Schizoaffective Disorder, Insomnia and Stress induced symptoms, as well as co-occurring disorders. We service Tempe and the greater phoenix metropolitan area.

Transitions Center was founded and is managed by a triple Board Certified psychiatrist of high repute who has lived and worked in the Phoenix area for over 30 years. Our providers excel in providing compassionate and individualized care while treating patients of all ages with different types of psychiatric disorders and alcohol and substance abuse issues. The center follows evidence based clinical guidelines and a holistic approach which leads to the most effective treatment of individuals suffering from various psychiatric disorders.

Bahoo A. Ghafoor M.D.

Board-Certified Psychiatrist & Medical Director
Dr. Bahoo Ghafoor is the Medical Director of Transitions Center. He is board certified by the American Board of Psychiatry and Neurology in Psychiatry, Child & Adolescent Psychiatry, and Addiction Psychiatry, and provides care across the lifespan — for children, adolescents, and adults — addressing both mental health and substance use disorders.
He earned his medical degree from the Geisel School of Medicine at Dartmouth and completed his general psychiatry residency at California Pacific Medical Center in San Francisco, followed by a fellowship in Child & Adolescent Psychiatry through the Creighton University School of Medicine program at Valleywise Health and a fellowship in Addiction Psychiatry at the University of New Mexico.
Dr. Ghafoor offers medication management with supportive therapy for children, adolescents, and adults, with particular focus on co-occurring psychiatric and substance use disorders, autism spectrum disorder, oppositional defiant disorder (ODD), and PTSD. He also serves as an attending psychiatrist on the inpatient adolescent unit at Aurora Behavioral Health in Tempe.
He is fluent in English, Urdu, and Punjabi.

Bahoo Ghafoor M.D.

Dr. Ghafoor completed his undergraduate degree in Physiology and a minor in Near Easter Studies from the University of Arizona.  He then attended Dartmouth Medical School (now known as the Geisel School of Medicine).  He completed his residency at California Pacific Medical Center in San Francisco.  Dr. Ghafoor’s passion for psychotherapy began in his residency as he learned how to perform therapy with patients struggling with a variety of mental illnesses.  Dr. Ghafoor then completed a fellowship in Child and Adolescent Psychiatry at the Creighton University School of Medicine – Phoenix program.  During this time, Dr. Ghafoor enjoyed working with very young children (0 to 5 years old) as well as adolescents struggling with Substance Use.  Dr. Ghafoor then completed a fellowship in Addiction Psychiatry at the University of New Mexico with a focus in working with adolescents who struggle with Substance Use.  

Dr. Ghafoor is currently triple board certified by the American Board of Psychiatry and Neurology in General Psychiatry, Child and Adolescent Psychiatry as well as Addiction Psychiatry. He is currently seeing patients from ages 3 to 18 years old.  Dr. Ghafoor can evaluate and diagnose Autism Spectrum Disorder, although some cases may require referral to a clinic that can perform a full Neurodevelopmental Evaluation with validated scales.  Dr. Ghafoor and diagnose as well as treat ADHD and a variety of Substance Use Disorders.  Dr. Ghafoor often incorporates supportive therapy with his medication management as he strongly believes in treating the whole patient as opposed to just prescribing medication.

He is the Past President of the Arizona Psychiatric Society (2012- 13), and an Assistant Clinical Professor at the University of Arizona, College of Medicine Phoenix.

Mani Murugesan, PA-C

Mani graduated with honors from Tamil Nadu Medical University in India, majoring in Physical Therapy, and earned a Master’s in Physical Therapy with a specialization in Neurological Rehabilitation from Oakland University in Michigan. His interest in medicine led him to Phoenix, Arizona, where he completed a second Master’s in Physician Assistant studies at Northern Arizona University.
Before joining Transitions Center as a psychiatric PA, Mani practiced in Primary Care and Physical Medicine & Rehabilitation. He focuses on treating adolescents (ages 14 and up) and adults experiencing psychiatric and/or substance use disorders.
Outside of work, Mani is a traveler at heart who enjoys experiencing different cultures, and is an avid motorcycle rider.

She has experience treating a range of psychiatric disorders in adolescents and adults (ages 12 and up). Yarmoni is dedicated to collaborating with her patients in a safe, welcoming environment, ensuring each person feels heard and leaves with the tools to improve their mental health.
Outside of work, Yarmoni enjoys good food and music, hiking, painting, making others laugh, and anything involving water.

Through personal and professional experiences, Yarmoni saw the impact of mental health on society and understood the need for mental health providers. As a result, she obtained a Doctor of Nursing Practice degree with an emphasis in Psychiatric Mental Health. Yarmoni has experience working with various psychiatric disorders in adults and adolescents above the age of 12. She is dedicated to educating and collaborating with her patients to achieve a mutual goal and ensure the best possible outcomes. She is committed to providing a safe and welcoming environment for patients to heal and gain the necessary tools to improve their mental health. Ethan is a board-certified Physician Assistant with a passion for mental health. He earned a Bachelor of Science in Biological Sciences from the University of California, Irvine, and a Master of Science in Physician Assistant Studies from A.T. Still University in Mesa, AZ. During his training he developed a focus on mental health, engaging in the assessment and management of psychiatric disorders in both outpatient and inpatient settings.
Ethan is experienced in treating adolescents and adults, and has a special focus on caring for members of the LGBTQIA+ community.
Outside of work, Ethan enjoys cooking, traveling, spending time with friends, and hanging out with his two cats, and is always exploring more of what Arizona has to offer.

Hi, I’m Rowan! I’m a Psychiatric Nurse Practitioner with a Doctor of Nursing Practice from Rush University in Chicago and a Bachelor of Science in Nursing from Alverno College in Wisconsin. I started my nursing journey in 2013 and have worked in a variety of settings, caring for patients of all ages. As a bedside nurse for seven years, I gained experience working with children as young as two with complex medical conditions and autism.

I discovered my passion for behavioral health while working at the University of Arizona Hospital in Tucson, where I supported adults and seniors through some of their most challenging times. That connection with my patients inspired me to return to school and specialize in mental health.

Now, as a Psychiatric Nurse Practitioner, my goal is to help people feel supported and empowered on their mental health journeys. I truly love what I do and feel grateful to be part of my patients’ stories.

When I’m not working, I’m spending time with my five-year-old, who keeps me on my toes and constantly teaches me new things about life. I also love relaxing with a good book and a great cup of coffee.
